Objectives:To examine the effect of an erectile dysfunction drug on the risk of death in patients who have taken sildenafil citrate (Viagra) over placebo. Twenty-two elderly patients with impaired renal function were randomized to take sildenafil citrate, the placebo, or a combination of sildenafil, placebo, or a combination of sildenafil and placebo. The primary endpoint was the incidence of a fatal or nonfatal heart attack. The secondary endpoints included the incidence of a fatal or nonfatal stroke, a cardiovascular death, a stroke, or a cardiovascular failure. This study was funded by the AstraZeneca Pharmaceutical Research and Development Fund, the UK Medical Research Council and the Royal Research Society (Research Fellowship).

The incidence of a fatal or nonfatal death was not significantly reduced from a mean of 1.3% at baseline to 0.2% at week 12 and then 0.1% at week 24 (p = 0.26).
Conclusion:In this double-blind, placebo-controlled, double-dummy study, the incidence of a fatal or nonfatal heart attack was reduced from 1.4% to 0.5% and from 1.0% to 1.3%, respectively. This reduction was not associated with the increase in the incidence of a fatal or nonfatal stroke.
The authors of the AstraZeneca Pharmaceutical Research and Development Fund also commented: “Our data suggest that a dose of sildenafil citrate, the generic name for Viagra, may be associated with an increased risk of fatal or nonfatal heart attacks and/or stroke in patients who have taken sildenafil, the generic name for Viagra, over placebo. This finding is consistent with a recent clinical trial, the Efficacy and Safety of Sildenafil in Patients with Chronic Obstructive Pulmonary Disease, which demonstrated that sildenafil, the generic name for Viagra, is more effective in the treatment of patients with chronic pulmonary hypertension (CPH) than placebo.”References
